Online appointments
If there is an urgent situation, call 070 – 345 07 09 (option 1). If there is a life-threatening situation, call 112.
In the evenings, at night and at weekends, as usual, in urgent situations you can contact the general practitioner HADOKS, tel. 070 – 346 96 69.
We would also like to point out the possibility of using the patient portal Uwzorgonline. You can make an appointment online or ask a question via this portal (e-consultation). In addition, you can view your lab results or request repeat medication via this patient portal.

Registration
Patient stop for new patients
In practice, no new patients are accepted with the exception of:
- People who are going to live together, of which one of the partners is already registered in our practice and family expansion. In that case you can use the registration form
It is not yet clear when the practice will be able to accept patients again. As soon as we accept patients again, you can see this on this website.
Deregistration
If you change your GP or if you move (abroad), please let us know. This way we can ensure your medical file doesn’t get lost and we can maintain an overview of our patient population. Furthermore, you are not allowed to be registered at more than one practice.
- Inform us in case you move or register elsewhere.
- Inform us if your contact details have changed (telephone number, e-mail).

Do you have a medical question and are you a patient with us?
You can submit your question online via the patient portal. If you don’t have access to the patient portal, then you first need to register once. For registration and more information click here.
If you prefer to speak to the assistant by phone, you can call the assistant (070 – 345 07 09 , option 3). In an emergency, choose option 1. If there is a life-threatening situation, call 112.
Appointments
Make an appointment by phone
In order to organize your and our time as efficiently as possible, we only work by appointment. To make an appointment or to request a home visit, you can call the assistant between 08:00am and 11:00am (070 – 345 07 09, option 3). Do you have multiple complaints or questions? Indicate it when making the appointment, we can take it into account in our agenda.
Make an appointment online
For less urgent complaints, or if you want to discuss persistent complaints with your doctor, you can also use our online appointment system. You must register here once as a patient, after which this data remains available for our electronic services only, such as requesting a repeat prescription online.The GP needs about 10 minutes per complaint, so if you have psychological complaints or multiple complaints, it is wise to make a double appointment. Then there is plenty of time for you.
Home visit
If you are not physically able to come to the practice for consultation (due to health reasons , age or physical handicap) please call the assistant and discuss the option of a home visit (preferably before 10am).Please realize that the opportunities for adequate care are more optimal in the practice and that not all services can be offered at your home.
No show policy
Appearing late or not appearing to your appointment can have various understandable reasons. Unexpected situations will naturally occur from time to time, and we fully comprehend this. However, if it happens too often we will unfortunately have to charge you. Our goal is not to punish, but to minimize the amount of time slots that go to waste.

COVID-19
For you and our safety and to protect our vulnerable patients, we ask you to observe the following measures during your visit to our practice:
- Keep 1,5 meters away.
- Come alone as much as possible.
- Do you need guidance? Then you can come with a maximum of 1 supervisor.
- Come up to 5 minutes in advance of your appointment.
